Social Worker - Recovery Team - Permanent

Key Responsibilities
- Complete strengths-based assessments and develop outcome-focused recovery and support plans
- Work with adults, carers and partner agencies to coordinate timely, joined-up interventions
- Manage a varied caseload, balancing risk, safeguarding and promoting independence
- Support people to access community resources, equipment, services and informal networks
- Maintain accurate records and contribute to reviews, meetings and service improvement activity

What You'll Need
- Social Work qualification (Degree/Diploma) and Social Work England registration
- Experience in adult social care, recovery/reablement, mental health and/or community-based teams
- Strong knowledge of safeguarding, the Care Act and risk assessment within multi-agency settings
- Confident communication, professional curiosity and a commitment to strengths-based outcomes
