Support Worker

Integrated Health Care Management is recruiting a Support Worker for Hart House in Southend-on-Sea, Essex. This is a full-time, rewarding opportunity to support adults in an open supported living unit staffed 24/7, where your work directly helps people build safer, more independent lives.

Key Responsibilities
- Deliver person-centred, recovery-based support plans with maximum service user input
- Support daily living, social and practical skills development to promote independence
- Contribute to risk management, reporting, and consistent documentation across shifts
- Carry out ongoing assessments and reviews to reflect changing needs and goals
- Work collaboratively with colleagues and partner services to maintain safe, positive outcomes

What You'll Need
- Experience supporting people with complex needs, including dual diagnosis (mental health and substance misuse) and/or forensic histories (desirable)
- A calm, professional approach with strong communication and de-escalation skills
- Confidence with record-keeping and following care, safety, and risk procedures
- A genuine commitment to recovery-focused support and working respectfully with individuals
